Nevada State Law requires that a student enrolling at a school have a record of immunization at the time of registration.

A copy of the statute is included in this section for reference to requirements, conditional enrollment, and exemptions to the law.

If an immunization record is not presented at the time of registration, it is the parent's responsibility to obtain a copy before the student can be enrolled in a class.  The parent assumes the responsibility for contacting the previous school or health care provider to obtain immunization records that reflect the current status.  A fax copy is acceptable for registration.

A copy of the immunization record is kept in the student's health folder and student's cumulative file.  It is attached to the Humboldt County school District Certificate of Immunization; a computer generated printout of immunization dates for DPT, OPV, and MMR.    This copy is imprinted with the Official Immunization Record Stamp of the Humboldt County School District.

If a student withdraws from the district, a copy of the immunization record may be provided to the parent for enrollment purposes at the next school.

The Health Folder including the immunization record of the withdrawn student will be forwarded to the next school following that school's request for records and a signed parental statement allowing release of records.

NON-COMPLIANT IMMUNIZATION RECORDS

The nurse will review the immunization record with the parent and explain what is needed for compliance with Nevada State Immunization Law.  A written statement of the needs will be provided to the parent.

10 YEAR BOOSTER DOSE OF T.D.

Nevada State Law requires that a student received proper booster doses or compliance with established immunization schedules.  Students must receive a booster dose for tetanus and diphtheria 10 years after the completion of the series required for school enrollment.  This booster will be due the last day of the monthes10 years following the month and year date of the school entry dose.

The parent will be notified of the need for the T.D. booster dose and the date for compliance.  Non-compliance with this requirement will result in the student's suspension from school.

The nurse provides the names of students how are non-compliant with the immunization law to the Principal.  The Principal sends written notice of suspension to the parent of the student.  The student is re-admitted to school when the parent provides an immunization record of the required T.D. booster.
